LPN - Adult Foster Care

About the Role:
KDC is seeking a dedicated Licensed Practical Nurse (LPN) to support adults enrolled in our Adult Foster Care (AFC) program. Working under the clinical supervision of an AFC Registered Nurse, the LPN will assist with health monitoring, caregiver support, documentation, and coordination of care. This role is ideal for a nurse who enjoys community-based work and building strong relationships with participants and caregivers.
Schedule: 20-32 hours/week, Monday-Friday...FLEXIBLE
Compensation: $30-33/hour
Required Qualifications:
- Current Massachusetts LPN license in good standing.
- Minimum of 1–2 years of nursing experience, preferably in community-based, long-term care, or human services settings.
- Knowledge of or interest in working with individuals with disabilities, chronic medical needs, or older adults.
- Strong communication, organizational, and interpersonal skills.
- Ability to work collaboratively under the supervision of an RN.
- Proficiency with Electronic Health Records (EHR) and basic computer skills (Microsoft Office).
- Valid driver’s license and reliable transportation.
- Ability to complete required trainings, including CPR and First Aid.
Preferred Qualifications:
- Experience in Adult Foster Care, home care, shared living, or community-based programs.
- Familiarity with MassHealth/Medicaid or other state-funded service systems.
- Bilingual or multilingual skills.
Key Responsibilities:
- Assist the AFC RN with clinical monitoring and follow-up for program participants.
- Conduct routine health check-ins and report any changes in participant condition to the supervising RN.
- Support the implementation of individualized care plans developed by the RN.
- Provide education and guidance to caregivers related to daily health needs, wellness, and medication observation (as appropriate within LPN scope).
- Participate in home visits as assigned and document observations accurately.
- Coordinate with caregivers, case managers, and internal team members to support participant well-being.
- Maintain timely and accurate documentation in the Electronic Health Record (iCentrix).
- Ensure services align with agency policies and regulatory requirements, escalating concerns to the RN as needed.
- Serve as a supportive clinical resource while adhering to LPN scope of practice and supervision guidelines.
